Privacy statement
This is the privacy statement of the Practice Musculoskeletal Medicine. We attach great importance to the protection of your personal data. In this statement we want to give you clear and transparent information about how we handle personal data.
Laws
The Practice of Musculoskeletal Medicine is bound by the privacy legislation, including the General Data Protection Regulation (AVG) and the Medical Treatment Contracts Act (WGBO) in the Netherlands and respects this. In general, this means that we handle your personal and medical data carefully and that we do not share them with third parties.
Our use of personal and medical data
The information we collect consists of: name, address, postal code, place, date of birth, telephone/mobile, general practitioner, citizen service number, the reason for coming into practice and related matters, general medical data and sometimes the name of the health insurer. This data is used for practicing the profession in individual health care such as:
- processing in the medical file
- determine and implement good medical policy
- avoiding serious health damage
- make contact with the person concerned
- financial settlement of a consultation/treatment
- informing relevant care providers such as the general practitioner, but only after consent of the person concerned as should be evident by means of a note in the medical file
- comply with laws such as mandatory reporting of certain infectious diseases.
When dealing with the data we will
- treat all data confidentially
- process your personal data in accordance with the purpose for which they were provided
- limit the processing of your personal data to only those data that are minimally required for the purposes for which they are processed
- protect the stored data against unauthorized use by means of passwords and/or lock and key
- not to provide data to third parties, unless with the written consent of the person concerned
- make the medical data available to the person concerned on request, for example for inspection, changes, additions, destruction or otherwise
- keep medical records for 15 years from the last treatment (this is an obligation)
- keep medical data longer if there is an important reason for this at the discretion of the practitioner.
